Yes, I've seen a more complete version of this, where the pilot states that he forgot to adjust his altimeter. More traditional altimeters (as opposed to RADAR and GPS based) work on air pressure and give readings above sea level. They also can give adjusted or secondary above-ground-level readings, but they need to be adjusted manually BEFORE take-off. While you are still on the ground, you wind a knob on your altimeter to adjust for zero. This pilot says he forgot to do that! Knowing what the USAF is like, he problem never sat behind the controls of a USAF aircraft again. He disgraced himself, his team, the USAF, the US DoD and the US itself.
